Return to the basics of medical care
Who am I? Where do I come from? Where am I going? When it was first established in April 2012, Zhuo Zheng also faced such "three questions in life." In order to find the answer, a start-up team of only six or seven people at the time spent three days and two nights discussing the topic.
What kind of medical institution is Zhuo going to become? For the start-up team that was still young at the time, answering this question was not easy, and the discussion was sometimes lively and sometimes fell into silence. "I don't know what I want to become, so why don't we talk about what I don't want to become first?" Someone suggested. The discussion immediately became smooth, and what Zhuo was about to become naturally appeared in front of everyone: providing objective diagnosis and treatment suggestions based on the interests of customers without excessive treatment; advocating team collaboration and pursuing excellent diagnosis and treatment capabilities; patiently listening and communicating, providing personalized diagnosis and treatment; rationally using drugs and using high-quality drugs; providing a comfortable medical environment to reduce waiting time; respecting customers and protecting customer privacy; Pay attention to the psychological needs of customers and family members... These seven consensuses discussed in three days and two nights have become Zhuo Zheng's core service philosophy, and Zhuo Zheng started from here. Nowadays, when you walk into any Zhuozheng house, you can still see them on the wall in a prominent position at the entrance.

Let patients return to their original roots
We believe that medical care should be a special industry that must balance commercial value and social responsibility, and cannot be operated simply as a "business." If you don't have awe for this industry, blindly pursue commercial interests regardless of other things, and ultimately won't be able to go on for long or continuous, such examples are everywhere. In Zhuo Zhengren's own words, we hope that Zhuo Zheng can become a model of the perfect unity of commercial value and social value in China's medical industry, and become a respected private medical institution. We must make money standing, make money selectively, and make clean money! The above are our thoughts when we set out, our daily practice in the past, and our guidance for taking every step in the future... Choosing medical treatment is equivalent to choosing long-distance running,"returning to the origin of medical treatment", and Zhuo Zheng will always be on the road!

So, what kind of medical institution does Zhuozheng want to become? As time goes by, the answer gradually becomes clear: in terms of quantity, there are not many medical institutions in China, but in terms of service quality and experience, medical institutions that can truly provide high-quality, standardized and reliable services are scarce, and medical institutions that are truly "patient-centered" are also scarce, and medical institutions that are truly respectable are even more scarce... Zhuo is about to become a medical institution that "returns to the basics of medical treatment"!

Let doctors return to their roots
Let medical institutions return to their original origins
"Sometimes I heal, often I help, and always I comfort." This epitaph by Dr. Edward Livingston Trudeau expresses the humanistic essence of medicine. We firmly believe that doctors should focus on performing their duties as doctors. Their treatment plans should be based on the interests of the patients, only prescribed drugs that need to be prescribed from a medical perspective, and only performed examinations that need to be done, rather than based on medical institutions. Treatment plans are formulated under the pressure of income-generation and the doctor's personal profit-seeking goals... To this end, we have become the first medical institutions in China to actively practice evidence-based medicine Evidence-Based Medicine, one of the medical institutions is committed to eliminating excessive treatment that patients hate. Our recruitment, training, assessment, and motivation of doctors are all focused on their professional abilities, so that they can be doctors with concentration and dignity, and become good doctors recognized by patients.

Practice complete evidence-based medicine
introduction
Dr. Li Tao Zhuo Zheng Medical Chief Medical Officer, Peking Union Medical College, Master's Degree, Anderson Cancer Center, USA, visited and studied former oncologist at the General Hospital of the People's Liberation Army of China
Just after the National Day of 2012, the opening of Zhuo Zheng's first clinic was on the agenda. Colleagues encountered questions when reviewing the introduction of drugs: What principles should be followed in drug selection? After a lapse of 13 years, I still remember the drug that caused the first internal debate: "Berberine Hydrochloride Tablets (Berberine)", which is an active ingredient extracted from traditional Chinese medicines such as coptis chinensis, phellodendron, and Sandichen. It belongs to a chemical drug. In vitro tests have been found to have a certain inhibitory effect on Shigella and Escherichia coli, and is widely used in gastrointestinal infections. The discussion process has been forgotten, but I still remember my final opinion: a large proportion of gastrointestinal infections are caused by viruses, so most cases can be cured through symptomatic treatment. Even bacterial infections can be replaced by better evidence and safer antibiotics, so it is not recommended to add berberine to the Zhuozheng drug list. This may be the first time in my memory that Zhuozheng was handling daily affairs based on evidence-based medicine principles.
evolution
In Zhuozheng's development process, the management team is most concerned about medical quality, but the diagnosis and treatment behavior measured by medical quality is both a "process" and a "result." Evidence-based medicine emphasizes making diagnosis and treatment decisions based on the best evidence, combined with clinical experience and patient wishes. Medical quality requires the provision of safe, effective, patient-centered, timely, efficient and fair medical services. Therefore, high-quality medical services must be based on evidence-based evidence. If not based on evidence, treatment options may be inefficient or even harmful, directly reducing the quality of medical care. There is an example here about children aged 4 and under not using cough and cold medicines. This principle was first proposed by the US FDA on the grounds that the efficacy of all children using cough and cold medicines is not different from other non-pharmaceutical methods (such as saline nasal washing and enough water), but the former causes significant increase in toxic side effects in children. The American Academy of Pediatrics immediately included this principle in its guidelines. However, this principle was not fully implemented in the United States at that time. A survey at the time even found that the proportion of cough and cold medicines used in clinics in the United States after the promulgation of the guidelines (2009-2010) was higher than before the promulgation of the guidelines (2005-2006).[1]. At the beginning of its establishment, Zhuozheng Pediatrics did introduce limited cough and cold medicines. However, when it studied that the risks of such drugs to children under the age of 4 were significantly higher than the benefits, the medical committee at that time still made this challenging topic the topic of the pediatric monthly meeting. At the meeting, I first introduced supporting data for the above views and proposed that Zhuozheng should completely stop cough and cold medicines for children under the age of 4. The atmosphere at the scene immediately became fierce. There are supporters and opponents. The most discussion focuses on what should we do if parents still want to take medicine after all the evidence is told. Although it was late at night, the final consensus was that parents needed not a drug, but a treatment to ensure that it was safe and effective. Although this communication was very difficult, since we all chose to use evidence-based medicine as the basic principle of medical decision-making, we must do this job well. Therefore, Zhuo Zheng is not proud to be the first medical institution in China to announce that it will not use cough and cold medicines for children aged 4 and under. Instead, after immediately removing all relevant drugs from the shelves, every doctor strives to explain the truth to parents in his own way and provide practical operation plans, so that the children treated by Zhuo Zheng can successfully survive every respiratory infection under the promise of safety. It also allows many parents to reduce unnecessary medication anxiety.
core concept
Many people and even medical staff are biased against the concept of evidence-based medicine. Some people think that evidence-based medicine is one piece of "evidence" after another that guides everything in medical behavior, but forget that evidence-based medicine emphasizes "best evidence"; Others think that the guide is an "order" and that doctors and patients have to follow it appropriately, but ignore that every item in the guide is written with "recommendation" in front of it, and it also needs to combine clinical experience with the patient's values and expectations; Others think that only doctors need to pay attention to evidence-based medicine. In fact, nursing, rehabilitation, psychology and even medical management personnel also need evidence-based thinking. Only when an interdisciplinary team works together can improve the overall quality of care.

Therefore, every medical staff entering Zhuozheng will receive education about what evidence-based medicine is: the core task of medicine is to maintain patient health, and evidence-based medicine is the ideal way to achieve this goal. The series of courses will teach you how to judge the quality of evidence, the correct accumulation of clinical experience in medical decision-making, and the values and personal preferences of treating each patient with respect. Under the influence of this core concept, we can see that as simple as Zhuozheng Pharmacy does not have any liver-protecting drugs or expectorants, because there are no truly effective ingredients in these two fields, or most blood collection items do not require fasting, because eating or not has no effect on most items; complex as Rapid Postoperative Rehabilitation (ERAS): Surgical patients with optimized path management before surgery (reducing anxiety, reducing fasting time, etc.)-surgery (minimally invasive, optimization of analgesic drugs, etc.)-surgery (early getting out of bed and eating, etc.) can return to normal in the shortest possible time. State, these results include the efforts of the three parties of medical, medical and technical parties to practice the core concepts of evidence-based medicine. We have truly used evidence-based medicine to solve patient problems.
vision for the future
Today's AI is affecting all aspects of human life. I think it is both a challenge and an opportunity to the development of evidence-based medicine. Evidence-based medicine has been developed for more than 40 years, but there are still many imperfections, such as significant differences between subjects and the real environment., and the non-repeatability of test results can lead to mistakes in medical decisions. If we can use AI in-depth search to conduct rapid and thorough data screening, and at the same time, with the help of AI, we can more objectively evaluate the pros and cons of each clinical decision to benefit patients, the prospects will be full of hope. I am looking forward to working with like-minded people to achieve this bright future.
